Appointments
For all appointments and inquires, please call us at (631) 486-8855. We are happy to answer any questions you may have, and look forward to helping you and working with you. Our regular business hours are Monday through Friday, 9:00am-5:00pm. Every effort is made to accommodate emergency appointment requests. On the day of your first appointment, please arrive 15 minutes early for registration and to fill out a medical history questionnaire. Please bring in any completed forms if you received them in advance.

> Driver’s License or other equivalent ID
> An accurate list of your medications, medical history, and allergies
> Any recent tests for your current problem (X-ray, MRI, CAT scan, EMG)
> Referral letter if required by your insurance

Insurance Information: > Dr. Leo works with most insurance carriers. He is an in-network, participating provider with some insurance plans, but has found it necessary with some carriers to care for their patients from outside of their provider network (out-of-network), in order to provide the best care and service possible. More recently, most health plans have been shifting a greater percentage of the cost of care to the patient, with higher deductibles and larger coinsurance responsibilities. We urge you to call your insurance carrier so you understand services that are included and are not included by your health plan. Our office is also happy to assist you in obtaining this information. We make every effort to work with all plans, including out-of- network carriers, and look forward to helping you with your hand care needs.

We value and respect your time, and we work hard to run on time during our office hours. Occasionally emergencies or patients with complicated medical problems require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.
